Recording an incident

This is the register after something has happened — an accident, a near miss, a safeguarding concern or property damage — not the risk assessment you write before the activity.

  1. Log it on Safety

    Open Incidents in the left-hand nav, New Incident. Choose type (Accident, Near Miss, Safeguarding, Property Damage), severity, date, location and a description, and who was involved. Accidents can record injury and treatment; property damage can take a description and estimated cost.

  2. Flag what has to be reported onwards

    Reportable circumstances are the POR Chapter 7 prompts — fatality, police or a statutory authority, significant media attention, or a significant number of people. The form then asks whether the relevant authority has been notified (National Safety Team, safeguarding, RIDDOR and so on, depending on what you ticked). Analyse can help draft; Create Incident saves it.

  3. Evidence and follow-up

    Open the incident for Photos, Witnesses (name, statement, contact) and Follow-up Actions with a due date, then Mark Complete as they're done.

  4. Move it through to closed — or up to district

    Start Investigation, then Mark Reviewed, then Close Incident. High or critical incidents also get Escalate to district. Filter the register by type, severity or status when you come back to it.
